The Office of the Conflict of Interest and Ethics Commissioner has developed several forms to support the administration of the Conflict of Interest Code for Members of the House of Commons.
Members are required to make a number of disclosures to the Commissioner. They can use these forms to confidentially disclose their private interests and those of their families, notify the Office of any material change to the information contained in a disclosure statement, and publicly declare sponsored travel and gifts or other benefits.
